Health Plans

Premier Choice Cardiology participates in many health plans.

When calling for your appointment, please let the receptionist know under which plan you have coverage with. Some plans require referral forms from your primary care physician. If this is the case, it is essential that the referral form is properly filled out and is brought to our office at the time of the visit.

If you do not belong to a health plan we participate in, you may still chose to follow up with us but in these case payments are directly collected from you. We file all insurance claims for you.
